owner/operator of the physical source of the spill. -Estimated volume of the spilled substance.
-Movement of the spilled substance; both the present direction of flow and the anticipated direction of flow.
-Containment and recovery actions, if any.
2-440.13 REPORTING
It is the responsibility of the owner/operator of the source of the spill to, within 10 days following the event, file a written report to the Authority’s Deputy Director of Environmental Engineering Services at (813) 870-8773. The written report shall describe all aspects of the event, including the spill discovery, initial spill report, cause of spill, identity of the spilled material, volume of spilled material, containment and clean-up actions, final disposal of the spilled material and any substances contaminated by the spilled materials, and recommendations for corrective actions to minimize the potential for the recurrence.
In addition, the owner/operator must comply with state/local

regulatory discharge reporting requirements in Section 62-
761.820 Florida Administrative Code (FAC), and take appropriate action within the three-day time period specified if the release is determined to be a reportable quantity (RQ), a copy of FDEP Form no. 62-761.900(1), must be faxed or delivered to EPC within 24 hours of the discovery of the discharge or before the close of the next business day.

FDEP Form no. 62-761.900(1) must be used to report any confirmed discharge, or of any one of the following conditions, unless the discharge is from a previously-known and reported
discharge:
Results of analytical or groundwater, or soils contamination by: field indicating tests of the surface presence water, of
a. b. A hazardous substance; A regulated substance; or
c. Petroleum products’ chemicals of concern specified in FAC, Chapter 62-770;
A spill or overspill event of a regulated substance to soil equal to or exceeding 25 gallons, unless the regulated substance has a more stringent reporting requirement specified in CFR Title 40, Part 302;
Free product or sheen of a regulated substance present in/on surface or groundwater, soils, basements, sewers, and utility lines at the facility or in the surrounding areas.
Soils stained by regulated substance observed during a closure assessment performed in accordance with FAC, Rule 62-761.800.

If the investigation of a suspected discharge indicates that an actual discharge did not occur, then a follow-up letter of retraction should be sent to the FDEP, with a copy to the EPC, within 14 days with documentation that verifies that a discharge did not occur.
2-440.14 CLEAN-UP AND REMEDIATION ACTIVITIES
The owner of any spilled hazardous substances and other regulated materials is responsible for the immediate clean-up and remediation of such substances. Personnel will cooperate with the recommendations from the Incident Commander of the TFR or ARFF units arriving at the spill scene.
