officer, flight engineer, and four non-revenue passengers were not injured. Visual meteorological conditions prevailed. The scheduled
domestic cargo flight, was being operated on an instrument flight rules plan under the provisions of Title 14 CFR Part 121.
The flight, to Memphis, Tennessee, was originating at the time of the accident.
According to a FedEx representative, at 0646, flight 840 "blocked out" from the FedEx ramp at DEN for a scheduled direct
flight to Memphis. The aircraft taxied approximately 6 miles, and at 0717 it was cleared for take-off on runway 16R. During
acceleration, the crew received a Take-Off Warning Horn and initiated a Rejected Take-Off. During
deceleration, the DEN tower reported via radio, that smoke was coming from the left gear. Immediately
thereafter, the tower reported that fire was coming from the same area and they initiated a call to
alert aircraft rescue and firefighting equipment (ARFF) to the scene. The aircraft came to a stop on
the runway centerline at the D3 intersection. After completing the appropriate checklists, the three
crew members and the four non-revenue passengers disembarked the aircraft via the evacuation slide.

The aircraft, on a cargo (newspaper) flight from Buenos Aires to Corrientes, crashed approximately
two minutes after takeoff from runway 17 at Ezeiza Airport. The pilot had reportedly
requested a return to the airport immediately before the aircraft lost control and crashed into a
field.

On October 16, 2003, about 1712 eastern daylight time, an Airbus Industrie A319-112, N764US, registered to and operated
by U.S. Airways, Inc., as a Title 14 CFR Part 121 scheduled domestic passenger flight, from Tampa, Florida, to
Pittsburgh, Pennsylvania, had the flight attendants injured while the aircraft was taxiing for takeoff at Tampa. Visual
meteorological conditions prevailed, and an instrument flight rules flight plan was filed. The pilot, copilot and 101 passengers
were not injured. Two flight attendants received minor injuries and one flight attendant received serious injuries.
The airplane was not damaged. The flight was originating at the time of the accident.
The Director of Flight Safety for U.S. Airways stated that the airplane was being taxied for
takeoff at Tampa International Airport, Tampa, Florida, when the airplane's steering system
failed, causing it to veer to the left. He further stated that the captain applied "abrupt"
braking to prevent the airplane from departing the taxiway, and all three flight attendants,
who were standing at the time, were injured, one of whom received serious injuries.

On October 29, 2003, at 0853 mountain standard time, a Cessna 208B, N791FE, operated by Corporate Air, Inc., Billings, Montana, as
Airspur 8773, was substantially damaged when it impacted U.S. Highway 14 while circling to land following a VOR-A instrument approach to
Yellowstone Regional Airport (COD), Cody, Wyoming. The airline transport certificated pilot was fatally injured. Instrument meteorological
conditions prevailed for the scheduled domestic cargo flight being operated under Title 14 CFR Part 135. An IFR flight plan had been filed for the
flight that departed Natrona County International Airport (CPR), Casper, Wyoming, approximately 0730.
According to witnesses, the airplane was observed to turn onto base leg abeam runway 04 approximately 200 feet above the ground.
The right wing dropped sharply, followed by the left wing dropping sharply, then the airplane rolled to the right, went inverted, and impacted the
highway. The airplane slid down an embankment and into Alkali Lake, coming to rest 40 feet from shore.

On November 1, 2003, at approximately 1205 mountain standard time, a Boeing 737, N956UA, was not damaged following a turbulence
encounter while on approach to Denver International Airport, Denver, Colorado. The airline transport pilot and airline
transport first-officer, 1 flight attendant, and 92 passengers were not injured; however, 1 flight attendant was seriously injured and 1
flight attendant received minor injuries. The airplane was being operated by United Airlines, as flight 1232, under Title 14 CFR
Part 121. Visual meteorological conditions prevailed. The airplane was on an IFR clearance from Boise, Idaho, to Denver, Colorado.
The pilot said that they were descending through flight level 240 when he instructed the flight attendants
