17 flight attendants, and 335 passengers were not injured. The flight was en route as a nonstop
flight to Hong Kong, China. Visual meteorological conditions prevailed, and an instrument
flight rules (IFR) flight plan had been filed. The crew reported that they suspected a tail strike
on takeoff. They dumped fuel and returned to land at San Francisco. Evaluation of the damage
resulted in structural repair to two structural members.

The aircraft, on a cargo flight from Khartoum to Wau, crashed short of the runway and exploded. Witnesses reported seeing the
aircraft on fire before it crashed into a farm field approximately 1.5 miles short of the runway.

Transport 0 - 0 - 3
The Airbus, owned by European Air Transport and operated on behalf of DHL, was hit by a SAM-7 surface-to-air missile shortly
after departure from Baghdad. The left wing caught fire following the hit, forcing the crew to turn the plane around for an emergency
landing back at Baghdad Airport.
11/29/03 Brussels, Belgium Boeing 747-258C Hydro Air Cargo 0 - 0 - 9
Hydro Air Cargo's Boeing 747 ZS-OOS was operating on a cargo flight from Brussels to Johannesburg
with an intermediate stop at Lagos. The aircraft crashed while attempting to land on
runway 19R. This runway, a 3900 x 60 meters asphalt runway, was closed to traffic because it
was being re-surfaced.
The aircraft, operating on a passenger flight between Boende and Kinshasa, crashed while attempting its third takeoff attempt after
two previous aborted takeoffs. The Antonov overran the runway, and crashed into a market square.

11/29/03 Denver, CO Boeing 737-300 Frontier Airlines 0 - 0 - 135
On November 29, 2003, at 0840 mountain standard time, a Boeing 737-300, N303FL,
operating as Frontier Airlines Flight 567, experienced a momentary lock up of the captain and
first officer's aileron and flight spoiler controls, while on final approach into Denver International
Airport (DEN), Denver, Colorado. The captain, first officer, 3 cabin crew and 130 passengers
were not injured. Visual meteorological conditions prevailed. The scheduled domestic passenger
flight was being conducted under the provisions of Title 14 CFR Part 121. An IFR flight plan had
been filed for the flight that departed Milwaukee, Wisconsin.
The captain reported that they were on final approach approximately 1/4 mile from the
end of the runway when he put in right aileron to correct for a crosswind. He had to use excessive
pressure to get the control yoke to respond. An uneventful landing was made at DEN. The captain
stated, "The yoke felt like it had bound up. [I] had to use excessive pressure to get the yoke
to turn to the right." After landing, he cycled the yoke left and right. After a few cycles, it seemed
to free up. The captain said they taxied to parking with the number 2 engine shut down.

SA227-AT Ameriflight 0 - 1 - 1
On November 29, 2003, at 0801:26 Pacific standard time, a Fairchild-Swearingen SA227-AT, N439AF, registered to Ameriflight, Inc., being
operated as Ameriflight flight 1966, and being flown by an airline transport rated pilot, was destroyed during collision with trees and terrain while
on final approach to Felts Field, Spokane, Washington. The pilot was fatally injured and a post crash fire consumed much of the aircraft. Instrument
meteorological conditions existed at the crash site and an Instrument Flight Rules (IFR) flight plan was in effect at the time. The flight, a
scheduled weekly (Saturday only) cargo trip, departed Boeing Field, Seattle, Washington, at 0707 PST, and was operated under 14 CFR 135.
The aircraft was cleared for the ILS/DME runway 21R approach and a radar track showed it turning right to intercept the localizer and
track inbound. Approximately 0801 the local controller at Felts Field issued a low altitude alert to the pilot who immediately responded acknowledging
a descent through 2,800 feet above mean sea level (MSL). The last radar target showed the aircraft tracking 217 degrees magnetic at 131
knots and reporting an altitude of 2,400 feet. The target was approximately 250 feet east of a tree line. The aircraft impacted the tops of a northsouth
line of conifer trees approximately 2,455 feet MSL coming to rest approximately 1,000 feet west-southwest in a drainage and about 3 miles
east-northeast of the airfield.
Felts Field is 1,953 feet MSL and the surface weather observation at 0753 reported winds from 230 degrees at 5 knots, a temperature
and dew point of 6 degrees Centigrade each, and a 400 foot overcast ceiling. The height of the ILS glide slope 2.615 nautical miles from the
