01. Parties to the investigation are the Federal Aviation Administration, Honeywell, M7 Aerospace, and Ameriflight, Inc.

On December 2, 2003, about 1645 eastern standard time, a Canadair CL-600-2B19, N916CA, operated by Comair Inc. as
flight 5242, was substantially damaged when it was struck by ground equipment, while standing at the John F. Kennedy International
Airport (JFK), Jamaica, New York. The certificated airline transport pilot, commercial co-pilot, a flight attendant, and 42
passengers were not injured. Visual meteorological conditions prevailed and an instrument flight rules flight plan had been filed for
the flight that departed Pittsburgh International Airport (PIT), Pittsburgh, Pennsylvania. The scheduled passenger flight was conducted
under 14 CFR Part 121.
According to the operator, the airplane was parked and in the process of deplaning passengers when it was struck by an
unoccupied belt loader. The operator of the belt loader fell off the belt loader as she maneuvered toward the airplane. The belt
loader struck, and substantially damaged the right side of the fuselage.
The weather reported at the airport, about the time of the accident included sustained winds of 25 knots, with 30 knot
gusts.

The aircraft was struck by lightning about 30 miles from the Bodoe Airport while on approach. The crew reported severe aircraft
handling problems, which resulted in one missed approach. Witnesses reported that on the second approach, the aircraft was porpoising
through the air, and crashed into the runway with a very high rate of descent. Two pilots and two passengers were aboard,
two of whom were critically injured.
12/14/03 Miami, FL Canadair CL-600-
2B19 Air Wisconsin 0 - 0 - 44
On December 14, 2003 at 1955 eastern standard time, a Canadair, CL-600-2B19 N445AW, registered to and operated by Air Wisconsin
as flight 2146 from Miami, Florida, enroute to Tampa, Florida, was struck by a pushback tug at Miami International Airport,
Miami Florida. The instrument commercial flight was operated under the provisions of Title 14 CFR Part 121 with a flight plan
filed. Visual weather conditions prevailed at the time of the accident. The airline transport rated pilot and his 40 crew and revenue
passengers were not injured. The accident occurred at 1955, on December 14, 2003.
During the pushback from the gate, the tug collided with the right side of the fuselage after the tug driver jumped from the
tug when the tow bar shear pin broke. The passengers were deplaned on the ramp thru the air stair door. Examination of the airplane
revealed 9 damaged ribs on the bottom side of the fuselage.

The aircraft, on a scheduled cargo flight from Oakland, California, crashed at 12:30pm local time while attempting to land on runway
36R at Memphis International Airport. For undetermined reasons, the freighter's landing gear failed, and the aircraft skidded
approximately 3,000 feet, off the runway, coming to rest at a 90° angle to the runway heading. The aircraft was consumed by fire.
One crewmember was hospitalized with broken bones -- the other six escaped uninjured.

The aircraft, on a cargo flight from El Yopal to Mitú, failed to arrive at its destination, and three hours later was declared missing.
The DC-9 freighter has yet to be located, and the three crewmembers aboard are presumed killed.

The aircraft, on a chartered passenger flight from Cotonou to Beirut, crashed while attempting to takeoff
on runway 24 at approximately 1415 GMT. Witnesses reported that the aircraft impacted a building, exploded,
and crashed into the sea. Weather at the time of the accident was good, with airport officials reporting
light winds and clear conditions. Various media outlets report that the plane may have been overloaded.

The aircraft [PR-GOO], on a São Paulo-Florianopolis-Porto Alegre flight, overran the runway while landing
at Florianopolis in poor weather. Conditions were reported as extremely windy, with heavy rain, at the
time of the accident. The 737 departed the runway surface and impacted a concrete brick wall, sustaining
